Notice on COVID-19 Vaccination

About the Vaccination

This vaccine is available free of charge for anyone at least 12 years old who wishes to receive it.

People who do not have a vaccine coupon (接種券/せっしゅけん) cannot get the vaccine.

When your vaccine coupon arrives in the mail, make sure to keep it safe. If you lose it, you will have to have another coupon issued.

  • Vaccination coupons were sent to people 65 years or older (born on or before April 1, 1957) on March 19.
  • For people 64 years old (born on or after April 2, 1957) down to 12 years old (born by July 1, 2009). your vaccine coupon was sent out on July 7.
  • People who turn 12 years old this fiscal year will be sent their vaccination coupon within one week after their birthday.
  • If you have not received your coupon when you should have, call or email the call center.

Start of vaccination and appointment availability varies by age and other factors.

  • The vaccine will begin being offered in May based on priority.
  • It is recommended that children 12-15 years old get vaccinated at a hospital or clinic.

It is necessary to get 2 doses of the vaccine.

Get the second dose as soon as possible.

  • Pfizer: 3 weeks (21 days) after the first dose
  • Moderna: 4 weeks (28 days) after the first dose

How to get Vaccinated

4. Make an appointment

- You cannot get a vaccine without an appointment.

A. Hospital/Clinic

Make a vaccination appointment with the hospital or clinic directly.

B. Vaccination Site

There are 2 ways to make an appointment:
1. Online, using the appointment website
(Available in Japanese only. Try to have someone who can understand Japanese help you fill out the form if you need support.)

5. Get vaccinated

- Go to the location at the time of your appointment. Do not forget to bring the following 3 items.
1. Vaccination coupon (接種券/せっしゅけん)
2. A form of identification (one of the following)
Driver’s license, passport, residence card, health insurance card, special permanent resident certificate, My Number Card, etc.
3. Prevaccination Screening Questionnaire
For questionnaires in English or other languages, use the translation as reference as you fill out the Japanese form (in Japanese as much as possible).

  • Children ages 12-15 must be accompanied by a guardian when getting vaccinated.
  • Try to have someone who speaks Japanese help you get your vaccine appointment and come with you to get vaccinated.
  • If you need an interpreter, call or e-mail the call center in advance.

6. Get the 2nd dose of the vaccine

  • It is necessary to get the 2nd dose of the vaccine as soon as possible 3 weeks (Pfizer) or 4 weeks (Moderna) after the 1st dose.
  • You do not need to schedule your 2nd dose. When you get your first dose, you will be told when your next appointment will be.

COVID-19 vaccination certificate for overseas travel

The vaccination certificate is issued to those who need to present it at inernational travel occasions. For other purposes,please utilize "Certificate of vaccination for COVID-19" or "Record of Vaccination for COVID19" which is issued when vaccinated.
